This publication proposes a plan for the creation of local palliative care programs in First Nations communities. It also presents a process to facilitate the evolution and development of this plan, as well as how-to guides, ideas, and lessons. All of these resources are drawn from the Improving End-of-life Care in First Nations Communities project, which aims to develop palliative care programs in four communities in Ontario.
